Apr 12, 2002 Dr. EE Internest Office Visit
Patient Medications: 1)Remeron 60 mg/day 2) Aspirin 81 mg/day 3) Lipitor 20 mg/day 4) Provigil 200 mg/day 5 Wellbutrin SR 450 mg/day 6) magnesium supplementation. Patient presents for follow-up of his chronic fatigue syndrome, probable depression and hyperlipidemia. He is happy because he has found a new psychiatrist. He has no other acute complaints. Exam is otherwise unchanged, but weight is up to 183.
Impression: 1) Weight gain - no doubt due to Remeron. 2) Fatigue syndrome stable 3) Depression - now on Provigil, but higher doses of Remeron and will have a psychiatrist adjust 4) Hyperlipidemia - stable on Lipitor
