Dec 18, 2002 Dr. EE Internest Office Visit
Medications 1) Zyprexa 2.5 mg/day - restarted 2) Celexa 60 mg/day 3) Wellbutrin SR 450 mg/day 4) Lipitor 20 mg/day 5) aspirin 81 mg/day. Tom presents for follow-up of his depression and is now back on Zyprexa and a higher dose of Celexa, but continues to do remarkably well. He has significantly fewer somatic complaints. Recent cholesterol is superb at 185 with an LDL of 96.
On examination he appears well and the rest of the exam is unremarkable with a BP of 130/72, weight is stable at 174.
Impression: 1) Depression - doing remarkable well. 2) Hyperlipidemia - stable on Lipitor.
Plan follow-up in 4 months.
