Impression:

Annual Exam

Medications as noted above


Patient presents for his annual exam and follow-up of his depression on multiple depressions, hyperlipidemia, and general health care as well as BPH.  He is urinating every 1 to 2 hours at night, but does not wish medications.  He also complains of some dribbling.  He denies fever, chills, chest pain, or shortness of breath, but is constipated from his medications.  His Geodon is now up to 100 bid and he is feeling well in general with increased energy.  He is working and has only minimal fatigue.


Medications: medications as note above and he has no hew allergies.  Review of system is otherwise unremarkable.  He resides by himself, does not smoke, and drinks minimally.  Recent blood tests show a cholesterol of 155, normal LFT's, normal glucose and a PSA of 2.98 with a normal CBC.


On examination he is a well appearing male whose weight is down to 173, BP 130/74, respiratory rate of 16, he is afebrile with a pulse of 70 and regular.
