EGD PROCEDURE REPORT


INDICATIONS:

The patient is a 81 yr old male here for an EGD due to History of Barrett's/HGC/cancer s/p endoscopic therapy.

PROCEDURE PERFORMED: EGD

w/biopsy

MEDICATIONS:

Nasal oxygen, MAC

TOPICAL ANESTHETIC:

none


DESCRIPTION OF PROCEDURE:

During intra-op preparation period all mechanical & medical equipment was checked for proper function. Hand hygiene and appropriate measures for infection prevention was taken. A physical examination was performed. After the risks, benefits and alternatives of the procedure and sedation were thoroughly explained, informed consent was verified, confirmed and timeout was successfully executed by the treatment team. The patient was anesthetized with topical anesthesia and the EG29-i10 (K111930), EC38-i1OL (A113168), and EG29-i10 (K111578) endoscope was introduced through the mouth and advanced to the second portion of the duodenum. The gastroscope was then slowly withdrawn and removed.


DUODENUM:

The entire examined duodenum appeared normal.

STOMACH:

A few sessile polyps were found in the gastric fundus.

ESOPHAGUS:

A torn hiatal hernia was noted. The mucosa of the esophagus appeared normal. Multiple biopsies were performed using cold forceps. Otherwise the examination was normal.


ESTIMATED BLOOD LOSS:

NONE

COMPLICATIONS:

There were no complications.

IMPRESSI8ONS:

   1. The entire examined duodenum appeared normal
   2. Few polyps were found in the gastric fundus
   3. 1cm hiatal hernia
   4. The mucosa of the esophagus appeared normal; multiple biopsies were performed
   5. Few innocent fundic gland polyps. Normal esophagus with nothing to suggest any neoplasis and no visible Barrett's nor esophagitis.

RECOMENDATIONS:

Await pathology results

REPEAT EXAM:

3 years EGD.

The attending physician performed the entire procedure.
