Internist, Dr. CC. Initial Visit: This gentleman was a patient of Dr. AA and comes in as a new patient since Dr. AA has closed his practice here. Patient is 51 years old and currently is unemployed, probably due to health reasons. Has chronic fatigue & depression. Has been under care of a psychiatrist, Dr. BB, for many years. Difficult for him to work because it is difficult for him to stay at a position for 8 hours without undue fatigue and emotional distress.
He is on Prozac 80 mg., Buspar 30 mg. daily, and occasional Librium. Recently he has been put on Mevacor by Dr. AA for elevated cholesterol at about 280, although he had a fair HDL. General health is good.
He really has no severe chronic medial problems. Past history is quite unremarkable. He has had a tonsillectomy. Broke a leg when he was a child. No serious illnesses; does have some hay fever and is allergic to a number of inhalants but no medications.
Habits: Doesn't smoke. Rarely drinks alcohol. Medications: As described.
Family History: Some history of possible diabetes, cancer of unknown type, and coronary disease.
Went over his records with him. Turns out that after he was put on Mevacor in January, his cholesterol drawn several weeks ago was 222, which certainly is an improvement. EKG in January was normal.
Evidently had some kind of a stress test [treadmill], but I can't tell from the tracings what actually was done.
Told him that it was fine for him to continue Mevacor 20 mg. daily, that I would like him to come in for a physical exam. Will get some initial lab data on a fasting basis and then check him over.
