Clinical History

Relevant History: elevated PSA
Submitting Diagnosis: IDE-9-CM:790.93:Elevated prostate specific antigen (PSA)

The patient is a 71-year man with history of elevated prostate-specific antigeddn. He now undergoes prostate biopsies.

Gross Description

The specimen is received in formalin in eight parts, each labeled with the patient's name and medical record number.


Part A, labeled "R. apex," consists of two 0.1 cm-thick cores of soft, tan-white tissue. The two cores each have a length of 2 cm and 1.8 cm. The specimen is entirely submitted in cassette A1. (rsm)


Part B, labeled "R. mid," consists of three 0.1 cm-thick cores of soft, tan-white tissue. The three cores each have a length of 2 cm, 1.2 cm and 1 cm. The specimen is entirely submitted in cassette B1 (rsm)


Part C, labeled "R. base," consists of two 0.1 cm-thick cores of soft, tan-white tissue. The two cores each have a length of 2.2 cm and 2 cm. The specimen is entirely submitted in cassette C1 (rsm)


Part D, labeled "R. anterior," consists of one 0.1 cm-thick core of soft, tan-white tissue. The one core has a length of 1.6 cm. The specimen is entirely submitted in cassette D1.

Part E, labeled "L. apex," consists of two 0.1 cm-thick cores of soft, tan-white tissue. The two cores each have a length of 1.2 cm and 1.1 cm. The specimen is entirely submitted in cassette E1. (rsm)


Part F, labeled "L. mid," consists of two 0.1 cm-thick cores of soft, tan-white tissue. The two cores each have a length of 2.2 cm and 1.8 cm. The specimen is entirely submitted in cassette F1. (rsm)


Part G, labeled "L. base," consists of two 0.1 cm-thick cores of soft, tan-white tissue. The two cores each have a length of 2.2 cm and 1.6 cm. The specimen is entirely submitted in cassette G1. (rsm)


Part H, labeled "L. anterior," consists of one 0.1 cm-thick core of soft, tan-white tissue. The one core has a length of 1.4 cm. The specimen is entirely submitted cassette H1. (rsm)



Diagnosis based on gross and microscopic examinations. Final diagnosis made by attending pathologist following review of all pathology slides. The attending pathologist has reviewed all dictations and preliminary interpretations performed by any resident involved in the case and performed all necessary edits before signing the final report.
