ABDOMINAL SONOGRAM: 11/27/07

CLINICAL DATA: Smoker. Evaluate for abdominal aortic aneurysm.


FINDINGS:

Sonograms of the abedomen were obtained with attention paid to the aorta.  The liver is seen to contain a few small cysts, the larger measuring 1.6 cm in maximal diameter within the lateral segment of the left lobe.  Also shown is a 0.8 cm diameter echogenic nonshadowing lession within the liver suggesting a small incidental hemangioma.  Hepatopetal flow shown in the main portal vein.  Normal-sized spleen measuring 8.5 cm in span.  Incidental note made of a 1.7 cm diameter moderately echogenic solid mass in the region of the right adrenal gland.  The gallbladder is incompletely distended without gallstones of focal tenderness.  Punctuate bright focus suggesting cholesterol deposition along the gallbladder wall.  No biliary ductal dilatation.  Extrahepatic bile duct measures 6 mm in diameter.  Where seen, the pancreas appears normal.  Calcific atherosclerotic plaque noted at the abdominal aorta without aneurysmal dilatation.  The aorta is noted to taper distally.


Renal lengths measure 11.1 cm on the right and 11.2 cm on the left without hydronephrosis or renal calculi.  No free intraperitoneal fluid  Enlarged prostate measuring 5.1 x 6.7 x 7.1 cm with estimated weight 125 grams.  Small to moderate volume within the urinary bladder.


IMPRESSION:

1.   Incidental note made of small hepatic cysts and 0.8 cm diameter echogenic liver lesion suggesting hemangioma.

1.   1.7 cm diameter right adrenal soft tissue mass.  Suggest further characterization by means of targeted CT scan.

3.   No hydronephrosis.  Marked prostatomegaly with estimated weight of 125 gm.

4.   No sonographic evidence of abdominal aortic aneurysm.
