Procedure Colonoscopy Proceedure Report November 18, 2015


Exam Date:November 18, 2015


Indications: The patient is a 73 year old Male here for a colonoscopy due to High risk for Colon Cancer. History of Colon Cancer.

PROCEDURE PERFORMED: Colonoscopy, diagnostic.

MEDICATIONS: Nasal oxygen, Midazolam (Versed) 1 mg IV and Meperidine (Demerol) 12.5 mg IV


DESCRIPTION OF PROCEDURE: During intra-op preparation period all mechanical & medical equipment was checked for proper function. Hand hygiene and appropriate measures for infection prevention was taken. A physical examination was performed. After the risks, benefits and alternatives of the procedure and sedation werte thoroughly explained, informed consent was verified, confirmed and timeout was successfully executed by the treatment team. The attending physician was present during the entire procedure. The EC-3890Li (A114875) endoscope was introduced through the anus and advanced to the surgical anastomosis. The scope was then completely withdrawn from the patient and the proicedure terminated.
Cecum withdrawal time: 7 minutes.


QUALITY OF PREP: Excellent


A digital rectal exam was performed and revealed revealed no abnormalities of the rectum. COLON FINDINGS: Tortuousd colon Anastomosis.
No polyps or masses. Retroflexed views revealed no abnormalities. Otherwise the examination was normal.


ESTIMATED BLOOD LOSS: None

COMPLICATIONS: There were no complications.

IMPRESSIONS: 1. Tortuousd colon Anastomosis. No plyps or masses. 2. Retroflexed views revealed no abnormalities.


RECOMMENDATIONS: High Risk Return in 1-2 years

REPEAT EXAM: Repeat exam in 1 year


The attending physician performed the entire procedure.
