CLININICAL INFORMATION: 74 yo man with right adrenal mass, pls evaluate for change in size.
COMPARISON STUDIES 5/27/07
FINDINGS: Real-time imaging of the abdomen was performed utilizing a multifrequency vector transducer.
Liver: The liver is normal in size, contour and echogenicity. No focal solid hepatic lesions are identified. 3.5 x2.0 x 2.7 cm cyst is noted in superior aspect of lateral segment of left hepatic lobe. It shows slight increase in size. (On prior examination it was 2.6 x 1.6 x 2.7 cm.) A second cyst measuring 3.1 x 2.0 x 3.6 cm with septations is identified in the inferior aspect of lateral segment of left hepatic lobe. This shows slight increase in size. (It was 2.1 x 2.1 x 3.6 cm on prior examination.,) The intrahepatic billary ducts are not dilated. The common bile duct measures 3.4 mm. Main portal vein measures 8.1 mm. Flow within the portal vein is hepatopedal.
Gallbladder: The gallbladder is normally distended. A 4 x 2 x 4 mm mobile stone is in the gallbladder. This is new from prior examination. The gallbladder wall is not thickened. No polyps are present. There is no fluid adjacent to the gallbladder. There is no tenderness to palpation over the gallbladder.
Pancreas: Due to overlying bowel gas, distal tail of pancreas is obscured.
Spleen: The spleen is normal and measures 6.4 cm.
Kidneys: The right kidney measures 10.2 cm and the left kidney measures 10 cm. The renal echogenicity appears normal. No cystic or solid renal masses are identified. There is no evidence of nephrolithiasis or obstruction.
Other<: No free intraperoneal fluid. The abdominal aorta is normal. The inferior vena cava is normal.
2.2 x 1.6 x 2.1 cm nodule is noted in the right adrenal gland. This shows no significant change when compared to the prior examination.
IMPRESSION:
1. 2 cysts in the lateral segment of left hepatic lobe, mildly increased in size from prior examination.
2. Small mobile gallstone, new from prior examination.
3. 2.2 x 1.8 x 2.1 cm right adrenal nodule, stable from prior examination.
4. Due to overlying bowel gas, distal tail of pancreas is obscured.
